A framing I keep running into - and want to push back on - is that urgent care is a symptom of our failed primary care system. It’s a clean rhetorical move, but I think it conflates something true (fragmented, transactional acute care as it’s commonly operated today is a real problem) with something false (urgent care itself is the symptom).

A few places I’d respectfully disagree:

1. “If primary care worked, urgent care wouldn’t exist” doesn’t survive the data. The systems with the most robust primary care on earth — Kaiser, Geisinger, the VA, the NHS — all still operate dedicated urgent care channels. Not because primary care failed, but because acute episodic care is a structurally different service line than longitudinal care.
2. Primary care, even when working well, isn’t built for episodic acute care. A laceration at 8pm, a febrile toddler on Saturday, a job-site wrist injury — these aren’t visits a PCP was going to handle well regardless of panel size. The training, staffing, equipment (X-ray, splinting, POCT), hours, and throughput cadence aren’t designed for unscheduled acute episodes. Asking primary care to absorb this volume is a category error.
3. The real comparator isn’t urgent care vs. primary care. It’s urgent care vs. the ED — where this volume used to go at 5–10x the cost. Urgent care didn’t pull patients out of primary care. It pulled them out of the ED.
4. The throughput critique is fair, but it’s a critique of how most urgent care is operated today, not of the model. Standalone retail urgent care is a real problem. Modern, integrated urgent care looks fundamentally different — same EMR as the PCP, warm handoffs, unattributed patients captured into the panel. Done well, it’s one of the highest-yield primary care acquisition channels a system has.
5. Models like Direct Primary Care are excellent for the slice they reach. They aren’t a realistic answer for 200M visits a year, Medicaid populations, FQHC catchments, or the family needing stitches at 9pm.

The honest reframe: urgent care isn’t a symptom of failed primary care. Disconnected, transactional urgent care is a symptom of failed integration. The fix isn’t to wish urgent care away — it’s to redefine its role so it strengthens primary care instead of substituting for it.

This is exactly the work [UCP Merchant Medicine](https://www.ucpmm.com/) and [Intellivisit](https://www.ucpmm.com/intellivisit-ai) have been doing for years — building the integrated urgent care model and the clinical decision support that makes acute episodic care consistent, connected, and evidence-based.

That’s the work. And there’s a lot of it underway.
